Great Plains Mental Health offers appointments via video conferencing through Zoom. This type of appointment is referred to as Telehealth or Telemedicine.

If your first appointment with GPMH is in Telehealth you  will need to complete the following:

1. Download New Patient Paperwork (call 402-614-0010 if needing alternate method of completion) 

2. Fill out forms completely 

3. Return forms along with a copy of your insurance card, photo ID, and if you have a COVID Vaccination Record. 

4. Fax to 402-614-0090 or Email to Contact@greatplainsmentalhealth.com

 

To initiate a telehealth appointment, inform your provider or call our front desk to get scheduled. Once scheduled, you will receive your appointment information via our portal or your email.
